Strategic Planning & Operational Excellence

Strategic planning is essential to driving priorities and delivering key results. PHAB offers 1:1 consultation to support the development of your organizational strategic plan and/or topic-specific strategic plans. Services are tailored to your needs and may include environmental scans; data collection from key stakeholders/staff; defining mission, vision, values, and priorities; and developing key objectives and project plans.

This full-day summit for staff and leaders provides a concentrated opportunity to develop a strategic plan mission, vision, organizational priorities, and key objectives and timelines. Substantive time is dedicated to hands-on, team-based work with guidance from PHAB experts. We end with a structured wrap-up to define next steps and ensure the work continues. This workshop can be designed for one or multiple organizations. For public health department clients, we also review PHAB Accreditation Standards & Measures.

Learn how to use PHAB’s Strategic plan template in this introductory training. Clients receive a facilitated walkthrough of the template, overview of key steps, and implementation guidance for their organization. For public health department clients, we will also review strategic plan-related PHAB Accreditation Standards & Measures.

Anticipate and prepare for shifts in policy, funding, and community priorities by exploring a structured set of scenarios and strategic implications. We partner with you to create a scenario planning framework, outline plausible futures, and determine potential adjustments and risk-mitigation strategies, culminating in a summary report to guide decision-making.

Population & Community Health Strategy

Identify your population and community health priorities, ensure alignment with standard requirements, and develop a strategy that advances your goals. This work can include an inventory of existing priorities and documents, an analysis of the internal and external landscape, stakeholder interviews or surveys, identification of strategic opportunities, and creation of a practical implementation roadmap with clear metrics to guide progress.
Develop an organization-wide or systemwide plan that embeds population and community health across operations. This includes assessing current structures and activities, defining shared objectives and milestones, clarifying cross-departmental roles and linkages, and recommending governance and resource alignment to support long-term- success.
CH(N)A/CHIPs help public health departments, health plans, hospitals, and other health care delivery organizations drive priorities and achieve key results to advance population health. PHAB offers 1:1 consultation to support the development of a CH(N)A/CHIP. Services are tailored to your needs and may include environmental scans, data collection from key stakeholders, defining a vision, and developing key objectives and project plans.
PHAB helps health care clients ensure value-based care goals are aligned with population and community health initiatives to optimize outcomes and financial incentives. Map existing value‑based care activities to population health priorities and identify alignment opportunities that improve efficiency and effectiveness.
We facilitate tailored workshops with executive leaders and boards to review population and community health priorities, explore organizational implications, and define next steps. Sessions include customized materials and facilitation guides, a summary of key insights and action items, and recommendations to support informed strategic decision-making.

Care Coordination

Develop care coordination infrastructure and plans for specific topic areas such as mental and behavioral health, chronic disease prevention and management, and maternal and child health. PHAB helps each client address best practices, analyze similar-sized health systems, define what strong care coordination looks like at their organization, assess current internal systems, and develop an implementation plan to improve coordination across internal departments and community health services. We also develop plans to support audits, internal KPIs, and other requirements aligned with improved care coordination.
